The Illinois Provider Directory for Children with Special Health Care Needs & their Families is a resource for both providers of specialized health care for children and the Illinois children and families they serve.  The goal of this directory is to enhance communication between providers of specialized health care for children and consumers.  The Internet provides a method for connecting both parties through the use of online directories like this one.

General Information:
You must have an email address to become a listed provider. Your email address will be your login name and all correspondence about your account will be done via email. (If you do not have an email account there are free services available. Among the popular ones are Microsoft's Hotmail and Yahoo.)

Completion of the application is a 2 step process - first you will be asked to provide information about yourself as a provider, including your area of specialization.  Then you will be asked to provide information about your office(s) and the services provided there. 

Once you have submitted an application, a brief review of  your entry will occur.  The information you provide will not be available for public viewing until it has been reviewed and approved. You will be notified via e-mail when the review process is completed and your application has been published for public availability.

You will receive a confirmation e-mail that includes a password to login to your account when your application is published.  This e-mail verifies your listing has been published for public viewing on our website.  Logging in to your account enables you to access your provider listing and expand or revise the information you want to make available to the public about your personal credentials, your services, or your office locations.

I further acknowledge and agree that I am a currently licensed health care provider in good standing and am accessing this site to either add or edit my free listing in the database.

I also understand and agree that if I attempt to add or alter this database for any illicit or improper reason, it is possible that I may be acting in violation of one or more state and/or federal laws, and if such improper action is so discovered, the web site sponsors shall be expected to turn over any and all evidence of such potentially illicit or improper acts to the proper state or federal authorities.
